Good news — we can now verify your identity electronically, making the process faster and easier for you. If for any reason we’re unable to verify it this way, we’ll ask you to provide physical documents from the list below.
If you cannot provide the documentation, please contact us.

Power of Attorney (POA) | |

We will need you to provide the following: | |
| An original or certified copy of the Power of Attorney document (certified on each page) or a Lasting Power of Attorney access code from the Office of the Public Guardian |
